This notice
This Notice of Privacy Practices describes how Chrisalys Surgery may use and disclose your protected health information (PHI) and how you can get access to this information. Please review it carefully. It applies to the practice at 436 N Bedford Drive, Suite 202, Beverly Hills, CA 90210, and to our workforce, including employed and contracted clinicians when they are working for this practice.
We are required by the Health Insurance Portability and Accountability Act (HIPAA) and applicable California law to maintain the privacy of PHI, to provide you with this notice, and to notify you if a breach of unsecured PHI occurs. We may change this notice. The current version will be posted on this site and available in the office. The effective date is the date this page is published unless a later date is stated below.

How we use and disclose PHI
Treatment. We use PHI to provide surgical and related care, to consult with other clinicians on your team (including voice therapy, mental-health letter-writers you have authorized, and covering surgeons), and to coordinate hair removal, recovery stays, and follow-up.
Payment. We use PHI to bill you, your insurer (including Medi-Cal, LA Care, IEHP, and private plans), and to obtain pre-authorization. That includes procedure codes, clinical notes reasonably necessary for the claim, and letters you have asked us to submit.
Health care operations. We use PHI for quality, training, accreditation, and running the practice (scheduling, our patient portal when access has been issued, and reviewing outcomes).
Other disclosures permitted or required by law. These include public-health reporting, health oversight, judicial processes, law enforcement as allowed, organ-procurement organizations when applicable, coroners, and workers' compensation. We may disclose PHI to family or friends involved in your care if you agree, or if you have an opportunity to object and do not. We will not discuss your gender-affirming care in a waiting room as entertainment. Marketing using PHI and sale of PHI require authorization except as HIPAA allows. Psychotherapy notes, when we hold them, have extra protection.
We will not use or disclose PHI for purposes not described here without your written authorization, which you may revoke in writing except to the extent we have already relied on it.
Your rights
You may inspect and obtain a copy of PHI we hold about you in a designated record set, with limited exceptions (for example, psychotherapy notes or information compiled for litigation). We may charge a reasonable cost-based fee as allowed by law.
You may ask us to amend PHI you believe is incorrect. We may deny the request in writing and you may submit a statement of disagreement.
You may request an accounting of certain disclosures, a restriction on uses or disclosures (we are not always required to agree, except we must agree not to disclose to a health plan for an item you paid for in full out of pocket, when the law requires that), and confidential communications (for example, a different phone number or no voicemail that names the practice).
You may receive a paper copy of this notice even if you agreed to receive it electronically.
To exercise these rights, write or call the practice. We will tell you if we need the request on a specific form.
Our duties and complaints
We must abide by this notice while it is in effect, train our workforce, and apply reasonable safeguards, including for email and portal messages. No electronic channel is perfectly silent; we will tell you when a method is not appropriate for highly sensitive attachments.
If you believe your privacy rights have been violated, you may complain to Chrisalys Surgery at (310) 247-4729 or in writing to 436 N Bedford Drive, Suite 202, Beverly Hills, CA 90210. You may also file with the U.S. Department of Health and Human Services, Office for Civil Rights (https://www.hhs.gov/ocr). We will not retaliate against you for filing a complaint.
