Facial Masculinization Surgery in Los Angeles

Facial masculinization surgery in Los Angeles

Jaw, chin, forehead, and thyroid cartilage, planned around the face you already have, not a template of angles.

The approach

Building structure without erasing you

Facial masculinization (FMS) is the counterpart to FFS: adding or rearranging structure so the face reads more masculine in ordinary light. That can mean a stronger mandibular angle, a wider or more projected chin, forehead contouring, a more prominent thyroid cartilage, and a nose that matches the new lower third. It is not a single operation, and it is not the same plan twice.

Dr. David N. Sayah, MD (ABPS, FACS) and Dr. Toby Mayer plan this work the way they plan FFS: unhurried, on your proportions, with implants or osteotomies chosen for tissue quality and how you want to age with the result. Hair restoration for beard or hairline can sit in the same conversation. See hair transplant when that is part of the picture.

Insurance coverage for FMS is real in California when the procedure is medically necessary gender-affirming care, but it is still plan-specific. We itemise a quote after consult. Financing for self-pay portions lives at /insurance/financing/.

Procedures

Procedures often in an FMS plan

01

Jaw and chin augmentation

Implants or bony work to widen the mandibular angle and project the chin so the lower third reads more masculine.

02

Forehead contouring

Reshaping the upper third so brow and forehead sit in a more typically masculine relationship to the eyes.

03

Thyroid cartilage enhancement

Building a more visible Adam's apple when that landmark matters to how you are read.

04

Masculinizing rhinoplasty

A stronger dorsum and nasal base that belong with the new jaw and forehead, not a separate cosmetic nose.

05

Beard and hairline grafting

Hair transplant where density supports the masculinized frame. Planned with the surgical dates, not afterthought.

A plan built for one person.

What to expect

What to expect.

01

Consultation

Photographs, goals, and a map of which thirds of the face actually need work.

02

Plan and quote

Implants versus osteotomy, staging, and an itemised estimate. Insurance pre-auth if you are using a plan.

03

Surgery

Often a single day for a combined plan; some patients stage jaw and forehead. You will know which before you book travel.

04

Recovery

Swelling is the honest part of FMS. We schedule follow-ups and, for travelers, a recovery stay if you want to heal here.

Common questions

Frequently asked questions

Is facial masculinization the same as FFS in reverse?

No. The anatomy and the implants are different. We do not invert an FFS checklist. The plan starts from your face and the landmarks you want people to notice.

Can FMS be done with top surgery?

Sometimes on separate dates. Facial swelling, a compression vest, and sleeping position all compete. We stack what is safe and stage what is not.

Does insurance cover facial masculinization?

Many California plans cover medically necessary FMS. Documentation and pre-authorization still apply. Start at /insurance or /insurance/pre-authorization.
