Hairline lowering in Los Angeles
Scalp advancement to bring the hairline forward, often with brow positioning, so the upper third of the face matches the rest of an FFS plan.
The hairline is part of the forehead, not a separate cosmetic
A high hairline lengthens the forehead. For many transfeminine patients, that length is one of the first things a stranger reads. Hairline lowering (scalp advancement) moves the hair-bearing scalp forward. It is frequently combined with a brow lift and with the bony forehead work of FFS, so the scar sits in the new hairline and the brows sit where they belong.
This is not the same as a hair transplant, though grafting can fill temples or hide a scar later. We decide which tool is right based on scalp laxity, hair density, and how much forehead bone is being contoured in the same sitting. Dr. Toby Mayer and Dr. Sayah plan this as facial surgery, not as an add-on at a hair mill.
When the procedure is part of medically necessary FFS, insurance may cover it. A quote after consult lists surgeon, anesthesia, facility, and whether grafting is a second stage. Travelers can often stack this with other facial work in one trip. See traveling patients.
Related upper-third work
Scalp advancement
Bringing the hairline forward within the limits of scalp laxity, with the scar designed to hide in hair.
Brow lift
Repositioning the brows so they sit with a feminized forehead rather than a masculine brow ridge.
Forehead contouring
Bony work that often happens in the same operation, which is why hairline and forehead are planned together.
Temple grafting (when needed)
Hair transplant to round the frontotemporal recession if advancement alone cannot.
What to expect.
Scalp and hair exam
Laxity, density, and how much lowering is realistic without a tight, shiny forehead.
Combined FFS mapping
Hairline, brow, and bone on one drawing. Grafting, if any, is scheduled as same-day or later.
Surgery and scar care
You leave with incision care, sleeping instructions, and a timeline for when hair can be styled over the line.
Frequently asked questions
Will I have a visible scar?
There is always a scar. The aim is a fine line inside or at the hairline. Density, skin, and how you heal decide how quiet it is. Grafting can help later if needed.
How much can the hairline come down?
Only as far as the scalp will move safely. We measure that in consult. Promising a number before that exam is not something we do.
Is hairline lowering covered with FFS insurance?
When it is part of a medically necessary feminizing forehead plan, many California plans include it. Verify at /insurance and complete pre-authorization if your plan requires it.
